Due to the impact of the COVID-19 pandemic, many provider offices may need to adjust their office hours or redirect patients to other offices. Some providers may also offer telemedicine visits as an option instead of an in-office visit. Members should always contact the provider’s office in advance to confirm availability.

Provider information contained in the First Health, Cofinity or First Choice of the Midwest online directories are updated weekly from information received from our provider network. This excludes interruptions due to system maintenance, upgrades or unplanned outages. The listing of providers in this directory is not a guarantee of benefits for the services they provide. Some hospital-based physicians such as anesthesiologists, pathologists, radiologists, neonatologists and emergency room physicians may not be network providers. Please check with the provider before scheduling your appointment to confirm he or she is participating in the network. We can't guarantee the availability of any particular provider. You may incur additional costs if you use out-of-network providers. Authorizations and referrals for care may be required to access some providers. If you have questions about authorizations, referrals or your health plan benefits, you can call the customer service number on your member ID card. Participating physicians, hospitals and other health care professionals are independent contractors and are neither agents nor employees of First Health. If an appropriate provider cannot be found within the directory, please contact your insurer/carrier to discuss arrangement of care from outside of the network.
